The TAP Admissions Team helps young adults who were in Child Intervention to transition into the Transition to Adulthood Program

The TAP Admissions Team supports young adults who were in Child Intervention to transition into the Transition to Adulthood Program. If you are under the age of 18 and already have a Child Intervention Practitioner, you don't need to apply for TAP. The Admission team will connect with your Practitioner soon after your 17th birthday to start the process. You can talk with your Child Intervention Practitioner about your goals and plan for your transition to adulthood.

As part of the TAP Admission process, the Admissions staff will book three meetings with you, your Child Intervention Practitioner, and your support network when you are 17, 17.5, and 18 years old. During these meetings, they will talk to you about your future hopes and dreams, what you want to do, and the kind of help you need.

If you are over 18 and think you may be eligible for TAP, and don’t currently have an open file you can send an email to [email protected] with your full name, date of birth, and where you live, and the TAP team will let you know if you qualify, and if you do, will get you connected to start the admission process.
